Logistical planning, such as managing appointments, PSW, utilizing the healthcare plan provided by the specialist and incorporating it into the lives of your caregiving team.

Supporting caregivers and providing easy tools for self care to mitigate burnout.

Initial Consultation:
We begin with a discussion about the your current needs, health situation, goals, and circle of care. This allows me to understand their specific challenges and identify areas where support is most needed.
Personalized Care Plan
I create a plan that outlines a step-by-step approach for attending appointments, organizing healthcare logistics, and supporting patients and families. Clients receive a clear structure and timeline for support.
Education
Discuss language, diagnosis, prognosis, stakeholders and their roles. To eliminate any assumptions, clarity everyones roles and expectations, and empower patients and families to make informed decisions which lead to patient INTEGRITY AND DIGNITY throughout their life regardless of diagnosis, prognosis or end of life choice.
Ongoing Support
I accompany patients to appointments, help them ask appropriate questions, and clarify any medical recommendations from doctors. Between appointments, I coordinate follow-ups, keep patients and families informed, and ensure all aspects of care are handled smoothly.
Summary & Next Steps
At the end of our engagement, clients receive a summary of healthcare progress, recommended next steps, and any additional resources for continued care and support. Patients and families feel assured and empowered, knowing they have a clear, compassionate advocate by their side.

“Understand your full situation”
Purpose: To provide a clear picture of the current care landscape for both the client and the caregiver. This step establishes the foundation for all future planning and support.
What’s Included
Home Visit: On-site assessment of the client’s condition and caregiver circumstances.
Initial Assessment Document: A detailed snapshot of the current situation and Next Steps (How Laipa can support you moving forward).
Presentation of Initial Assessment: It is recommended that family unit | Inner Circle are included in this meeting to discuss assessment and next steps.
Pricing & Payment
o Pricing: $750 (includes Home Visit, Initial Assessment Document and inner circle meeting)
o Payment Terms: 50% deposit due prior to the home visit. Balance payable upon completion and sign-off of the Initial Assessment Document.
o Refund Policy: This package is non-refundable once the assessment visit has taken place. Deposit valid for 4 weeks; if the assessment is not scheduled within that time, deposit is non-refundable.
Outcome
You receive a comprehensive written document capturing the full picture of the present situation. This becomes the foundation for designing a tailored Action Plan in the next stage.
When Done
When Initial Assessment Document is completed and signed off by client.
Two back and forth for modifications included, any further communications will be invoiced in addition to Package 1 Fee.

“Set up systems and supports”
Purpose: Executing a plan, to get off ground.
To bring your personalized Action Plan, as defined in the Initial Assessment, to life. This package focuses on setting up systems, supports, and communication so that care feels clear, coordinated, and manageable.
What’s Included
The list below shows examples of action items that may be implemented in Package 2.
The actual items will be determined by the needs identified in your Initial Assessment.· Care Team Activation: Setting up OH@Home coordination, arranging PSW support, and ensuring communication with providers.
· Medical Appointment Advocacy: Accompaniment to family doctor and specialist visits to ask questions, clarify instructions, and integrate medical guidance into the Action Plan.
· Safety & Support Systems: Organizing Lifeline emergency response, medication review, and other immediate supports.
· Inner Circle Communication: Clarifying roles, responsibilities, and communication channels within the support network.
· Caregiver Coaching: Practical coaching on boundaries, self-care, and communicating needs clearly.
· Community Connections: Recommendations and setup for day programs, grocery support, or Meals on Wheels.
2.1 Basic Package
Covers up to 5 action items.

2.1.1Pricing & Payment
o Pricing: $1000 (Covers up to 5 action items)
o Payment Terms: 50% deposit due at booking. Balance is payable once the agreed-upon action items are completed and signed off.

o Refund Policy: The 50% deposit is non-refundable. The balance is only invoiced once action items are completed and approved.
2.2 Extended Package
More than 5 action items.
Additional examples may include:
· Coaching | Communicating patient’s needs and desires to the inner circle and health team (e.g. PSWs, providers).
· Clarifying roles and responsibilities of the inner circle.
· Recommended supports such as day programs, grocery supports, or Meals on Wheels.
· Defining and documenting inner circle roles.
· Advocacy at Doctors appointments.

o Pricing: Custom Pricing for assessments more than 5 action items.

Coaching_1:1 Coaching: $125/hour (includes Summary) (4 × $125): $500
o Payment Terms: Custom pricing with 50% deposit due at booking. Balance payable once additional action items are completed and signed off.

“Ongoing care management”
Purpose
To step in as the manager of your care plan and provide ongoing “continued | operational care”. I take on the responsibility of coordinating, monitoring, and adjusting supports so that you and your family can focus on being present with one another.
What’s Included
Full Oversight of Care Plan: Acting as the central point of coordination for all healthcare providers, support services, and inner circle members.
Regular Communication: Weekly check-ins (as needed) and updates with the family and care team.
Medical Appointment Advocacy: Ongoing support at family doctor and specialist visits, including note-taking, clarification of recommendations, and follow-through with the care team.
Document & Information Management: Maintaining a secure, organized portal with care documents, updates, and action plans.
Decision Support: Helping families navigate choices with clarity, confidence, and dignity.
Advocacy & Representation: Communicating with providers on behalf of the client to ensure their needs and wishes are honored.
Ongoing Monitoring:
Monthly reviews of the Action Plan to ensure it reflects current needs.
Adjustments to supports and services as circumstances evolve.
Future planning with compassion — balancing hope for the best with preparation for what may come, while keeping peace, love, and kindness at the center of decision-making.
Ongoing coaching and support for clients and families as needed.

Pricing: Monthly Charge | Flat Fee
o Pricing:
Bronze: $1,000/month (Support for clients with steady, low-intensity needs. Regular check-ins, clear communication, and reliable coordination to keep care organized.)
Silver: $2,000/month (Support for clients with moderate or changing needs. Ongoing coordination, appointment advocacy, and consistent guidance for families navigating evolving care.)
Gold: $3,000/month (cSupport for clients with complex, high-intensity, or end-of-life needs. Close involvement, frequent coordination, and comprehensive advocacy for families facing significant transitions.)
Payment Terms: $3,000 retainer due at signing; monthly invoicing for ongoing services.
o Refund Policy: Retainer is refundable if services are discontinued. Monthly charges already invoiced are non-refundable.
Outcome
You have a dedicated Care Manager ensuring that the right supports are in place, communication flows smoothly, and your loved one’s wishes are always prioritized. This reduces stress, prevents gaps in care, and allows you to return to your role as family.

Defined time commitment is done. Every 4 weeks.

“Adjust when needs change”
Purpose
For returning clients whose circumstances have shifted and who now require a new Action Plan. Instead of starting over with a full Initial Assessment, this package provides a focused reassessment that captures the current situation and creates an updated action plan.What’s Included
o Review of the previous Action Plan and outcomes
o Reassessment of the client’s current situation and needs
o Updated documentation reflecting new priorities and supports
o Creation of a new Action Plan tailored to evolving circumstances
Executing the Change Request Assessment
This stage is about assessing a former client’s new needs and supports. This is a reassessment including a new Action item list. If client desires to have Laipa take action on the action items defined in the Change request assessment- refer to package 2 for details.
Pricing & Payment
o Pricing: Change Request Assessment $500 (plus cost of delivering of new Action Plan)
o Payment Terms: $500 due prior to Change Request Assessment.
o If the Action Plan requires implementation, Package 2 guidelines apply (50% deposit; balance due once agreed-upon action items are completed and signed off).
o Note: some actions may not be completed due to constraints - or situational changes.
o Refund Policy: This package is non-refundable once the assessment visit has taken place.
Outcome
You receive an updated, clear, and practical Action Plan that reflects the client’s new needs and supports. This ensures continuity of care without repeating the entire Initial Assessment process.
When Done
When Change Request Document is considered complete once it is signed off by client.
Two rounds of modifications are included; additional revisions or communications will be invoiced separately.

"Emergency Support when you can't wait"
Purpose
Urgent, hands-on advocacy and coordination services required when a returning client faces rapidly changing circumstances that cannot wait for the standard Change Request Assessment process. This service bridges the gap between assessment and implementation when clients can't wait for standard timelines.
When This Service Is Needed:
o Crisis situations where client's condition has suddenly deteriorated
o Urgent care transitions (unexpected hospital discharge, care provider changes)
o Family emergency requiring immediate advocacy support
o System failures (PSW no-show, OH@Home service disruptions)
o Time-sensitive decisions that must be made before full reassessment completion
What’s Included (not limited to the following)
o Real-time advocacy at medical appointments, hospital visits, or care meetings
o Emergency coordination with healthcare providers, OH@Home, PSW agencies
o Crisis communication with family members and inner circle
o Urgent documentation of changing medical status or care needs
o Immediate problem-solving to address care gaps or safety concerns
Pricing & Payment:
o Pricing: $250/hour for active coordination work
o Payment Terms: $1000 deposit required, with weekly invoices to reflect ongoing needs and changes
o Refund Policy: Deposit is non-refundable. Services already provided are billed at the agreed hourly rate
o Discounts: Friends & Family 50% discount available

You and your family receive immediate, hands-on advocacy and coordination during urgent or unexpected changes in care. This ensures safety, reduces confusion, and provides stability and reassurance until longer-term plans are in place.
